There is an addy on the back but form states about 3 TIMES that you must send in the original form :(. There is a telephone # though- maybe you can request one? It is 1-888-667-3774. I noticed these displays in a few stores today- all in the same place- front endcap, soda aisle and they are new, so I am sure it will make an appearance in your store soon.
On the Miami Dolphins website, it states that these are only located in the Miami area. Has anyone found them outside of Miami? Or is there anyone in Miami who could assist a Dolphins fan? I live in Fort Myers and every Publix I’ve been in only has Tampa Bay Bucs displays 🙁 Thanks!
